The Office of the Inspector General (OIG) report on Jeffrey Epstein's death in federal custody revealed severe lapses in protocol, negligence, and misconduct by Bureau of Prisons (BOP) staff at the Metropolitan Correctional Center in New York. Epstein, who was awaiting trial on federal sex trafficking charges, died of apparent suicide on August 10, 2019.
